Job Description

The Resident Services Coordinator will deliver personalized, resident-centered support by assessing individual needs, coordinating resources, and advocating on their behalf. Key responsibilities include case management, program planning, and collaborating with local agencies to promote independent living and overall well-being.

Work Schedule

  • Shifts: Monday, Tuesday, Wednesday, Thursday, Friday

Responsibilities

  • Assess resident needs: Evaluate physical, mental, social, and financial requirements to develop tailored service plans.
  • Monitor & Reassess: Continuously evaluate service effectiveness and adjust plans accordingly.
  • Eligibility & Application Assistance: Screen residents for program eligibility and support their application processes.
  • Information & Referrals: Provide residents and families with resources at local, state, and federal levels.
  • Family Communication: Engage with family members to gather information and encourage their involvement.
  • Program Planning: Organize monthly activities covering social, cultural, educational, and recreational needs.
  • Health Screenings: Coordinate health screening programs for residents.
  • Resource Coordination: Leverage NCC resources to meet resident needs effectively.
  • Collaborate with Staff: Work closely with NCC Extended Care Facility teams for comprehensive support.
  • Resident Advocacy: Serve as a liaison to advocate for resident rights and concerns.
  • Volunteer Development: Create volunteer programs and informal support networks.
  • Resident Respect & Communication: Treat all residents and families with courtesy, utilizing active listening skills.
  • Record Keeping: Maintain accurate charts, records, and documentation such as Vial of Life forms.
  • Resident Transitions: Facilitate smooth transfers to hospitals, nursing homes, or other institutions.
  • Quality of Life Inspections: Conduct inspections at least biannually.
  • Reporting: Maintain and submit timely reports on resident data and services.
  • Additional Duties: Perform other tasks as assigned by department leadership.
  • Resident Activities Support: Assist in planning and coordinating resident activities.

Experience & Skills

  • Educational Requirements: Bachelor's Degree in a social services-related field (preferred); Associates Degree required.
  • Language Skills: Bilingual in Vietnamese is a must.
  • Experience: Minimum of 1 year in the social services field.
  • Licenses: Valid NJ driver's license.
  • Knowledge: Familiarity with social work theories, practices, and procedures.
  • Technical Skills: Proficiency in Microsoft Office Suite and Google Docs.
