We are seeking a dedicated and compassionate Vietnamese-Speaking Community Health Worker to join our team. 🙋‍♀️ Duties

• Engage with Vietnamese-speaking community members to assess their health needs and provide culturally appropriate education on topics such as nutrition, medication management, and preventive care.
• Assist individuals with navigating healthcare systems, including understanding Medicare benefits and scheduling appointments.
• Conduct or attend outreach activities to promote health awareness campaigns and community events tailored to Vietnamese populations.
• Collect and document data related to community health trends, service utilization, and patient feedback using accurate record-keeping practices.
• Help communities find and enroll in health coverage through Medi-Cal or Covered California.

🎯 Qualifications

• Fluency in Vietnamese and proficiency in English are essential for effective communication within the community and with healthcare teams.
• Previous experience advocating for communities is desirable.
• Experience working with individuals with disabilities or in community outreach programs is desirable.
• Ability to work in a fast-paced environment and multitask with minimal supervision.
• Compassionate attitude with excellent interpersonal skills to build trust within the community.
• Computer literacy to navigate the internet, submit reports, and documentation online, with experience in Microsoft Suite (Outlook, Excel, and Teams).
