🔹 Position Overview

Reports To: Director of Social Work

Location: Queens/Manhattan, New York, United States

🔸 Responsibilities

• 🤝 Work collaboratively with clinical social workers to provide comprehensive social work services at service units
• 📝 Complete psychosocial assessments for patients as indicated
• 🎯 Formulate and implement tailored intervention plans for patients
• 📞 Offer concrete case management, follow-up, and referral services
• 🌐 Provide entitlements/benefits eligibility counseling and assist with applications
• 🤝 Support Clinical Social Workers in case management tasks as needed
• ⏱️ Ensure timely services in accordance with Social Work department guidelines
• 📄 Complete documentation and reports following departmental standards
• 💼 Attend case conferences, agency meetings, and staff meetings
• 🌍 Participate in community outreach and marketing activities
• 🔧 Perform other duties as assigned

🎓 Minimum Qualifications

• 🎓 Bachelor's Degree in Social Science, Social Work, Psychology, Sociology, or a related field
• 🧑‍💼 At least one year of experience in social services, health care, or a related area
• 🗣️ Good verbal and written communication skills
• 🌐 Bilingual in English and Chinese (Mandarin/Cantonese) required

• 💰 Salary: $50,000.00 - $55,000.00 per year
