Registered Nurse at Oak Street Health

About the Role

The primary goal of our Registered Nurse at Oak Street Health is to forge strong relationships with patients by coordinating comprehensive care and providing a seamless, supportive experience for patients and their care teams.

Using an integrated approach, you'll leverage standards, guidelines, and pathways to deliver effective care. Your clinical assessment, intervention, and education efforts will ensure patients receive timely, competent nursing care. As a vital member of the team, you’ll utilize data and information to improve health outcomes and foster an engaging, welcoming environment through team communication and delegation. 🤝

Our Registered Nurses typically report to the Practice Manager or Nurse Supervisor.

Core Responsibilities

• Provide safe, proficient nursing care aligned with job responsibilities and safety standards.
• Respond reliably to incoming calls, exceeding patient expectations with exceptional service.
• Manage triage and symptom assessment for both in-person and remote patients.
• Administer medication management using standardized protocols for prescriptions, refills, and prior authorizations.
• Review and update medication lists thoroughly and accurately.
• Deliver patient education and direct care around chronic conditions, via in person, phone, or group sessions.
• Monitor and collaborate on care plans for diverse patient panels.
• Build relationships with patients and caregivers to promote preventive care and avoid ER/hospital visits when appropriate.
• Create a welcoming environment tailored to patients, families, and communities.
• Delegate activities to clinical team members to meet patient needs.
• Participate in team meetings to discuss patient care and clinic operations.
• Ensure an exceptional patient experience through attentive service, responsiveness, and respectful care.
• Perform point of care testing, procedures, and specimen collection, including phlebotomy.
• Other duties as assigned to support clinic operations.

What We're Looking For

Required Qualifications

• Active Registered Nurse (RN) license in good standing within your state.
• BLS certification.
• Experience with Electronic Medical Records (EMR).
• Ability to maintain patient confidentiality at all times.
• Legal US work authorization.
• Strong communication and assessment skills, including remote patient evaluation.

Strongly Preferred Qualifications

• Experience collaborating with interdisciplinary care teams.
• Proficient computer skills with data analysis capabilities.
• Effective problem-solving and prioritization skills.
• 2+ years of healthcare experience as an RN.

Additional Preferred Qualifications

• Experience working in a clinical setting.
• Ability to work independently.
• Fluency in Spanish, Polish, Russian, or other languages spoken by community members.

Details & Compensation

Weekly hours: 40 hours

Position type: Full-time

Pay Range: $72,627.00 – $155,538.00

This range reflects the base salary or hourly rate, with actual offers depending on experience, education, location, and other factors. The role may also include bonuses, commissions, or incentives.
