What You Will Do

• Practice independently in the community as part of an interdisciplinary care team
• Conduct comprehensive patient assessments to determine needs, develop treatment plans, and set outcomes
• Deliver personalized rehabilitation therapy to patients in their homes or care facilities
• Continuously evaluate patient progress and adjust plans of care accordingly

Qualifications

• Minimum of one year of physical therapy experience or successful completion of probation as a VNS Health Rehab Intern
• Valid licensure to practice as a Physical Therapist in New York State
• Valid driver’s license required for areas outside Manhattan

Pay Range per Visit

If applying for the per diem, per visit position, the standard pay range is:

• All areas except the Bronx and parts of Brooklyn (Downtown Brooklyn, Williamsburg, Greenpoint): $70–$75 per visit
• Bronx: Weekday visits - $95 | Weekend visits - $100
• Brooklyn (Downtown Brooklyn, Williamsburg, Greenpoint): Weekday visits - $95 | Weekend visits - $100

Salary Range

Annual compensation ranges from USD $85,000.00 to USD $127,500.00.
