Responsibilities

The Patient Engagement Specialist (PES) plays a vital role in new patient acquisition, driving organizational growth and fulfilling engagement commitments to Harmony Cares' customers and partners. The role involves reaching out to eligible patients, explaining services, addressing inquiries, gathering feedback, and scheduling appointments. The PES must respond quickly, professionally, and compassionately to meet patient needs and support our clinical team.

Essential Duties and Responsibilities

  • Conduct outbound calls to eligible patients to facilitate initial in-home appointment scheduling 📞
  • Provide detailed explanations of program services to patients
  • Accurately document call notes and scheduling details within our electronic system
  • Address emergency clinical calls expediently when required 🚑
  • Process new appointments and modify existing appointments as needed
  • Capture and document patient and family feedback, including any special needs, to ensure high-quality care
  • Achieve and maintain departmental metrics such as Quality Scores, Acceptance Rates, and Patient Engagement numbers 🎯
  • Demonstrate compassion, attention to detail, and collaborative teamwork
  • Maintain professionalism and participate in call quality monitoring for continuous improvement
  • Use exceptional communication skills to clearly convey services and address patient needs 🗣️
  • Complete ongoing training and coaching for skill development within specified deadlines
  • Work collaboratively with corporate and regional team members to provide outstanding service
  • Perform additional duties as assigned

Qualifications: Required Knowledge, Skills, and Experience
