Overview

VillageCareMAX is seeking a motivated Outreach Coordinator to provide dedicated assistance to our members by coordinating services, promoting preventive care, and ensuring seamless communication. The ideal candidate is bilingual in Chinese (Cantonese/Mandarin) and has a passion for community health and service excellence. 🌇

Key Responsibilities

• Handle inbound communication: Answer calls and address questions from members and providers 📞
• Assist with access: Help members access covered and non-covered services aligned with their care plans 📝
• Engage members: Conduct QA conversations and confirm preventive care appointments ✅
• Coordinate appointments: Arrange visits with healthcare providers and facilitate transportation logistics 🚗

Qualifications

• Minimum of 3+ years of relevant experience
• Excellent communication, organizational, and problem-solving skills
• Bilingual in Chinese (Cantonese/Mandarin) is required
• Willingness to travel to the office and community events
