📝 About the Position

Girling Health Care of NY is looking for a dedicated bilingual Russian-speaking Registered Nurse to serve as an Onsite Intake RN, working out of Maimonides in Brooklyn. You will facilitate patient transfers from hospitals or skilled nursing facilities to home health care, ensuring compliance with all relevant laws, regulations, and agency policies.

🎯 Position Summary

The Nurse Liaison plays a key role in coordinating patient transitions from hospital or skilled nursing settings to home care, helping to ensure safe, effective, and compliant care delivery.

🔑 Key Responsibilities

  • Aligns with Agency Mission: Implements the agency’s philosophy, goals, and objectives within policy guidelines.
  • Community & Staff Education: Interprets and promotes the agency’s philosophy to both staff and the community.
  • Patient Assessment: Collaborates with hospital teams to evaluate home-care needs, safety, and discharge plans.
  • Patient & Family Interviews: Assesses understanding of illness, and evaluates home environment and support systems.
  • Education & Evaluation: Checks patient comprehension for procedures like insulin injections and dressings.
  • Medical Communication: Discusses hospital treatments and patient responses with medical staff.
  • Referral Screening: Reviews referral for appropriateness of care and compliance with criteria.
  • Patient Education: Explains home health services and policies to patients and families.
  • Insurance & Financial Coordination: Verifies coverage, discusses co-pays, and explains financial requirements for services (e.g., CMS face-to-face).
  • Coverage Assessment: Evaluates third-party coverage to ensure sufficiency for patient's needs and discusses additional resources if necessary.
  • Referrals & Equipment: Makes referrals for additional services and arranges for medical supplies or equipment.
  • Discharge Preparation: Ensures home health readiness by communicating patient needs to agency staff and arranging staff training on unfamiliar procedures.
  • Follow-up & Feedback: Provides post-discharge feedback to hospital or skilled nursing staff.
  • Team & Agency Participation: Participates in staff orientation, agency committees, and quality improvement activities.
  • Additional Duties: Engages in routine responsibilities, performance evaluations, and ongoing performance improvement plans.

🎓 Qualifications

  • License: Current New York State Registered Nurse license
  • Education: Bachelor of Science in Nursing preferred
  • Experience: 1-2 years in community health within a Certified Home Health Agency (CHHA) or Long Term Home Health Care Program (LTHHCP)

🧠 Knowledge, Skills & Abilities

  • Strong planning, problem-solving, and prioritization skills
  • Knowledge of Medicare, Medicaid, and commercial insurance regulations
  • Excellent interpersonal, leadership, and documentation skills
  • LANGUAGE: Must be bilingual in Russian
