Key Responsibilities

• Perform comprehensive health assessments and physical examinations in patients' homes 🏡
• Diagnose and manage acute and chronic health conditions ⚕️
• Develop and implement individualized care plans 📝
• Prescribe medications and treatments within the NP scope of practice 💊
• Order and interpret diagnostic tests 📊
• Provide patient and family education on health maintenance and disease management 👨‍👩‍👧‍👦

Requirements

Qualifications

• Education: Master of Science in Nursing (MSN) or higher from an accredited Nurse Practitioner program 🎓
• Licensure: Active New York State Nurse Practitioner License (Required) 🏛️
• Certification: Board Certified as a Nurse Practitioner ✅
