Job Description
The NP / PA is responsible for providing comprehensive telehealth services, including conducting virtual visits, conducting assessments, and coordinating patient care with a focus on reducing hospitalizations and improving health outcomes.
Essential Duties and Responsibilities
- Complete E/M visits for new patients, assessing their suitability for chronic care management and remote patient monitoring services.
- Obtain detailed medical histories, conduct virtual examinations, and perform risk assessments based on medical and behavioral health conditions.
- Recommend treatment plans for follow-up by nursing staff and the case management team.
- Provide patient and caregiver education by clear communication.
- Collaborate with other healthcare providers to optimize patient outcomes, aiming to reduce emergency room visits and hospital stays.
- Address escalated patient vitals and health measurements promptly.
- Offer clinical support through consultations, condition education, and intervention strategies focused on outcomes and best practices.
- Ensure organized, timely, and accurate documentation including progress notes, prescriptions, and evaluations compliant with policies.
- Engage with patients via outreach to ensure continuity of care and follow-up services.
- Perform crisis intervention when necessary to support patient stability.
- Handle any other duties as assigned to support patient care and clinic operations.
Required Qualifications
- Active NY state license as NP or PA with independent prescriptive privileges.
- Bilingual proficiency in Cantonese and English.
Skills and Experience
- Experience in virtual care / telemedicine.
- Background working with geriatric populations.
- Proven ability to provide competent clinical services to patients with chronic health conditions and disabilities.
- Experience with HEDIS/PECOS certification is preferred.
- Strong interpersonal, communication, and teamwork skills.
- Familiarity with harm reduction and motivational interviewing techniques.
