Position: Nurse Practitioner - Advanced Practice Provider (Bilingual Spanish Preferred)

Role Description
The Nurse Practitioner at Oak Street Health delivers effective, equitable, value-based primary care tailored for adults on Medicare. They see patients independently, collaborate with physicians based on individual needs, and foster long-term relationships that build trust and connection to care plans. 🩺
Practicing within outpatient settings, they are part of a collaborative care team that includes physicians, PAs, medical assistants, a dedicated medical scribe, and support staff like registered nurses and care managers. Our partnerships extend to specialists and hospitals for specialty and acute care, enabling providers to focus on care inside the clinic—primary care, care coordination, and population health.
Core Responsibilities
- Provide exceptional primary care.
- Conduct office visits for routine and acute issues.
- Administer annual wellness visits and health risk assessments with thorough documentation.
- Coordinate care with other providers, specialists, testing facilities, and agencies.
- Lead population health initiatives, such as ensuring eligible females receive biennial breast cancer screenings.
- Assist with phone triage and outreach efforts.
- Educate patients on health conditions, care plans, and treatments.
- Participate in promotional activities at Oak Street Health.
- Conduct home visits as needed.
- Perform other duties as assigned.
This role reports to the Center Medical Director and works closely with operational leadership.
Required Qualifications
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
- National certification in at least one of the following:
- Family Nurse Practitioner
- Adult-Gerontology Primary Care Nurse Practitioner
- Adult Nurse Practitioner
- Gerontological Nurse Practitioner
- Active, non-probationary state Nurse Practitioner license
- Active DEA license
- US Work Authorization
- Embodies the “Oaky” spirit
Preferred Qualifications
- Experience in primary care, internal medicine, geriatrics, or family medicine
- Knowledge of HCC documentation, ICD-10 coding, and HRAs
- Passion for teamwork and cross-disciplinary collaboration
- Interest in innovative, population health-driven models
- Bilingual proficiency in applicable areas

Time Type: Full time
Estimated Pay Range: $95,738 – $206,206 annually
Note: Actual salary offer depends on experience, education, location, and other factors.
