Job Overview

We are seeking dedicated professionals to work in our Community-Based Adult Services and Adult Day Health Care facility. Experience in ADHC is a plus!

Qualifications

Masters Social Worker (MSW):

• Masters in Social Work, OR
• Masters in Psychology, Gerontology, or Counseling with one year of social services experience, OR
• Bachelor's degree in social work with two years of social services experience.

Social Worker Assistant:

• Bachelor's Degree in Social Work, Psychology, Sociology, or Social Welfare from an accredited college or university.

Key Duties & Responsibilities

• Provide assessments and reassessments of participants 📝
• Develop and implement Individual Plans of Care
• Conduct individual and group counseling sessions for elderly participants as per each care plan
• Plan and coordinate discharge procedures
• Document all provided services accurately
• Deliver care management services, including assistance with IHSS and Access programs
• Assist with Primary Care Physician appointments
• Manage casework related to housing referrals and social welfare program access
• Complete all relevant paperwork and maintain participant files 🗂️

Compensation & Benefits

• MSW starting salary: $22/hour (DOE)
• Social Worker Assistant starting salary: $18/hour (DOE)
• Steady salary, vacation, health & dental benefits, and 401K plan
• Bilingual in Vietnamese required
