Job Overview

The Care Manager acts as the primary communicator between members and the Plan. Responsibilities include developing and reviewing person-centered service plans with members and their families, coordinating both covered and non-covered benefits, and delivering a positive member experience. This role emphasizes health education and maintaining ongoing, meaningful communication with members, at minimum monthly. 🩺✨

Essential Responsibilities

• Maintain ongoing communication with members, depending on individual needs, at least monthly.
• Follow up and respond promptly to items requiring attention.
• Provide health education to members, collaborating to improve and stabilize health, aiming to minimize hospitalizations.
• Coordinate care with members, their families, and vendor agencies.
• Create an excellent member experience by addressing concerns and fulfilling needs.
• Document thoroughly and timely in members’ charts.
• Participate in team meetings and supervision sessions.
• Perform other duties as necessary for the role.

Knowledge, Skills, and Abilities

• Understanding of member assessment procedures.
• Knowledge of clinical review processes.
• Strong grasp of health education.
• Effective communication skills with individuals of all ages and socioeconomic backgrounds.
• Ability to read, interpret documents and communicate effectively with members and staff.

Required Education, Experience, and Licenses

• Background in health care and/or long-term care.
• Hold an RN license or a Master’s Degree in Social Work.
• Bilingual skills in Mandarin or Cantonese are preferred.
• Hybrid schedule: 3 days in-office, 2 days remote.

Compensation

• MSW: $34.61 to $39.42 per hour
• RN: $41.82 to $50.48 per hour
