Job Overview

The Care Manager serves as the key liaison between members and the Plan. This role involves developing and reviewing person-centered service plans with members and their families, coordinating both covered and non-covered benefits, and ensuring members enjoy a positive experience. Additionally, the Care Manager provides health education and maintains ongoing communication with members, at a minimum, on a monthly basis.

📝 Essential Responsibilities

• Regular communication: Engage with members at least once a month, adjusting as needed based on individual requirements.
• Follow-up: Respond promptly to issues requiring attention.
• Health education: Educate members to promote health improvements, help stabilize conditions, and reduce hospitalizations.
• Care coordination: Collaborate with members, family members, and vendor agencies to coordinate care effectively.
• Member experience: Deliver excellent service, addressing concerns and needs proactively.
• Documentation: Maintain thorough and timely records in members’ charts.
• Team participation: Contribute to team meetings and supervision sessions.
• Additional duties: Perform other tasks as necessary to support the role.

🎓 Specific Knowledge, Skills, and Abilities

• Understanding of member assessment procedures
• Familiarity with clinical review processes
• Proficiency in health education principles
• Effective communication skills across all ages and socioeconomic backgrounds
• Ability to read, interpret documents, and convey information clearly

🎓 Required Education, Experience, and Licenses

• Background in healthcare and/or long-term care
• RN licensure or Master’s Degree in Social Work
• Bilingual skills preferred (Mandarin or Cantonese)
• Hybrid work schedule: 3 days in-office, 2 days remote

💰 Compensation

• MSW: $34.61 to $39.42 per hour
• RN: $41.82 to $50.48 per hour
