Role Overview
The Medical Social Worker is an essential member of Oak Street Health Centers, bringing a unique perspective to support the psychosocial needs of our patients. Collaborating with interdisciplinary teams, they will perform assessments, advocate for patients, and coordinate comprehensive care for high-risk individuals and their caregivers—aiming to improve health outcomes and reduce hospitalizations.
Core Responsibilities
- Bilingual Spanish required
- Support high-need patients through assessments, telephonic check-ins, and in-person visits.
- Partner with patients and their support persons to set goals, enhance engagement, address barriers, and implement care plans.
- Utilize community resources and make referrals to meet patients' healthcare and psychosocial needs.
- Navigate insurance benefits in collaboration with the Patient Relations Manager to optimize plan utilization.
- Provide ongoing and episodic care coordination to empower patients in managing their health and improving quality of life.
- Educate patients on supportive care options, including advance care planning and end-of-life care, along with goals of care counseling.
- Offer psychoeducation to patients and families on issues like dementia, illness adjustment, aging, and caregiver support.
- Collaborate with care teams to solve complex psychosocial problems and lead interventions where needed.
- Ensure timely documentation to keep the care team informed of intervention progress.
- Consult with Community Health Workers to address complex patient needs.
- Respond effectively to crises, prioritize tasks, and manage urgent situations.
- Other duties as assigned.
This role reports directly to the Practice Manager.
Qualifications We’re Looking For
Required Qualifications ✅
- Master of Social Work (MSW) from an accredited school (CSWE approved).
- State licensure or minimum licensure in the work location.
- Fluency in Spanish, Polish, Russian, or other languages spoken by community members (if necessary).
- Proficiency with computer tools including Microsoft Office, GSuite, and electronic medical records.
- Legal authorization to work in the U.S.
Strongly Preferred Qualifications ⭐
- Experience with complex care coordination or population healthcare management.
- Ability to utilize metrics and data to guide practice and improve outcomes.
- At least one year of medical social work experience in healthcare, ideally within the last two years.

- Experience working with multidisciplinary healthcare teams.

Compensation & Benefits 💰
The typical salary range for this role is $55,916 - $121,474. Actual offers depend on experience, education, and location, and additional incentives such as bonuses or short-term incentives may apply.
