Role Description

The Clinical Informatics Specialist (CIS or Medical Scribe) at Oak Street Health plays a crucial role in supporting primary care providers with clinical documentation, enabling them to focus on delivering outstanding care. Scribes assist throughout the patient care journey — from morning huddles to plan the day’s visits, joining providers in exam rooms to observe and document, and following up after visits to support next steps.

Beyond documentation, these team members serve as clinical documentation assistants, ensuring accuracy and specificity. They become experts in our value-based care model, chronic condition management, and coding (ICD-10 and CPT). Extensive on-the-job training covers clinical workflows, preventative care, population health data, and team-based care.

Ideal candidates should commit at least 1-2 years, making this an excellent opportunity for pre-med students or those pursuing careers in health informatics, public health, healthcare administration, medical coding, and related fields.

Responsibilities

  • Patient Encounter Documentation (80%)
    • Joining providers in exam rooms to observe patient visits
    • Documenting patient encounters in structured notes (history, assessment, plan, physical exam)
    • Assigning CPT and ICD-10 codes
    • Preparing After Visit Summaries
    • Consulting with providers for accurate documentation
  • Clinical Documentation Improvement (10%)
    • Requesting and reviewing medical records
    • Supporting documentation with population health tools
    • Assisting with daily huddles and documentation reviews
    • Collaborating with providers on documentation opportunities
  • Administrative Support (10%)
    • Placing orders and referrals
    • Handling administrative tasks
    • Supporting the care team with clinical documentation-related responsibilities
    • Performing other duties as assigned

What We're Looking For

Knowledge

  • Medical terminology and common medications (from pre-med or clinical experience) [required]
  • Prior clinical experience, including shadowing or volunteering [strongly preferred]
  • Scribe or transcription experience [preferred but not required]

Skills

  • Advanced listening and communication skills [required]
  • Strong computer literacy and quick learner of new workflows [required]
  • Fluency in Spanish, Polish, Russian, or other languages spoken by community members [required where indicated]

Abilities

  • Adaptability to new workflows and quick learning [required]
  • Typing speed of 70+ words per minute [strongly preferred]
  • Teamwork and willingness to follow provider directions [required]
  • Self-motivation within the role [required]
  • Excellent attendance and ability to work in-person during clinic hours [required]
  • Commit to at least 1 year in the role (2+ years preferred) [required]
  • Availability to work approximately 40-45 hours weekly during clinic hours [required]
  • Compliance with policies including HIPAA [required]
  • US work authorization [required]
