License Home Care Agency Is Hiring An Intake Specialist

🏢 About the Role
As an Intake Specialist, you will play a crucial role in coordinating patient intake processes, verifying eligibility, and ensuring smooth onboarding for our clients. If you are organized, proactive, and possess excellent communication skills, we want to hear from you!
🔹 Intake Specialist Responsibilities

Make introductory calls to newly referred members:
- Collect demographic information (Exact name, DOB, Address, SSN)
- Gather doctor’s details (Name, Phone Number)
- Obtain insurance information (Medicaid/Medicare Number, Secondary Insurance Name and Number)
- Educate patients on available services/programs (PCA, CDPAP, MAP) through MLTC
- Briefly explain the enrollment process
- Provide necessary forms for enrollment via mail, email, or fax
- Set submission due dates to facilitate quicker enrollment
- Enter information into HHAExchange and update referral status to In Progress
- Verify eligibility and benefits of Medicare, Medicaid, and Managed Care via E-paces
- Enter MLTCs under referral info and submit referrals to at least two MLTCs
- Follow up with MLTCs within 3-4 business days regarding RN visit dates
- Ensure appointments are scheduled within 30 days from the DOH or M11Q date; request new forms if needed
- Follow up with patients or doctors on required documentation
- Assist patients with scheduling Maximus appointments if necessary
- Remind patients of upcoming RN visits and prepare them for appointments
- Make follow-up calls to patients on the day of their appointments
- Confirm start of services with MLTCs
- Follow up a week before the start of services on authorization status
- Mark referral as admitted once authorization is received and notify the Coordination and Nursing Departments via email
- Update appointment calendars and coordinate meetings or appointments
- Perform administrative tasks such as filing, photocopying, collating, and faxing

- Excellent written and verbal communication skills
- Resourceful and proactive in addressing issues
- Exceptional organization skills with the ability to multitask and prioritize under pressure
- High school diploma or equivalent
- Fluent in English and Russian or Spanish
