Overview
As an On-Site Intake Representative, you will play a vital role in connecting patients with essential home care services. This full-time position offers weekly pay between $21.00-$23.00 per hour, with office hours from Monday to Friday, 8:30 am - 5:00 pm. Our office is located at:
148 39th St. Suite 19-4-BBBrooklyn, NY 11232

Responsibilities
What You'll Do:
- Receive referrals from referral sources
- Verify patients’ insurance coverage to determine eligibility
- Educate patients on available services
- Assist patients with scheduling assessments necessary to determine care eligibility, including NYIA (Maximus)
- Submit referrals to MLTC plans for potential enrollment
- Determine services approved by insurance providers
- Secure authorization for MLTC cases and provide case details to the HHA Coordination team

Qualifications
What You'll Bring:
- Strong communication skills, both oral and written
- Excellent interpersonal skills
- Intermediate computer skills including proficiency with Microsoft Office (Word, Excel, PowerPoint), HHAexchange, and Salesforce.com
- Ability to type quickly and accurately
Education and Experience 🎓
- High School diploma or GED required
- Minimum of two (2) years of experience in Customer Service or telephone sales
Physical Requirements 🧍♂️🧍♀️
- Sedentary work: ability to remain in a stationary position for extended periods
- Effective communication skills to clearly exchange information with others
