📝 Job Summary
As a Home Health Care Aide, you will deliver compassionate, tailored care to clients in their homes, supporting their independence and well-being. This role involves working closely with diverse populations, such as seniors, individuals with disabilities, or those recovering from illness, creating a warm and trusting environment.
🔧 Duties & Responsibilities
- Assist with Activities of Daily Living (ADLs) — bathing, dressing, grooming, toileting, and mobility support
- Monitor & document patients’ vital signs — blood pressure, pulse, temperature, and respiration — accurately using EHR/EMR systems
- Administer medications following prescribed guidelines, with strict HIPAA compliance for privacy
- Support clients with specialized needs such as Alzheimer’s, dementia, hospice, or developmental disabilities
- Operate medical equipment like Hoyer lifts for safe transfers and perform catheter care when needed
- Prepare nutritious meals and assist in meal planning to promote health
- Perform light housekeeping — cleaning, laundry, and maintaining a safe environment
- Maintain detailed medical records and ensure comprehensive care documentation
- Collaborate with healthcare professionals using EMR/EHR systems for seamless communication
- Provide emotional support & companionship to enhance quality of life
- Support hospice or end-of-life care, respecting dignity and individual wishes when required
🎯 Qualifications
- Speaking Hindi or Punjabi is required at this time
- Experience in home health care, assisted living, or nursing homes is highly valued
- Knowledge of medical terminology and procedures (vital signs, catheter care, CPR certification)
- Experience working with disabilities, dementia, or Alzheimer’s disease preferred
- Familiarity with EMR/EHR systems for documentation
- Ability to safely lift patients using assistive devices like Hoyer lifts
- Physical stamina for long shifts and ability to work independently following care plans
- Excellent observation and compassionate communication skills
- Flexibility for live-in schedules or varied shifts
- Understanding of HIPAA regulations for confidentiality
- Valid driver’s license and reliable transportation are preferred for community visits
