Job Summary

The Coordination Team Lead is responsible for overseeing the daily operations of the team. This includes coaching and training team members, ensuring policy adherence, maintaining interdepartmental collaboration, and fostering communication to deliver the highest level of patient care.

Responsibilities

  • Ensure all company policies and staffing protocols are followed
  • Oversee daily dashboard to confirm aides' attendance and patient safety
  • Manage a team of Scheduling Coordinators, review and approve PTO, complete performance evaluations, and maintain coverage during absences
  • Assist coordinators with challenging staffing cases, resolving complaints or incidents as needed
  • Assign cases to coordinators within the team
  • Serve as the main contact for all contracts
  • Monitor patient satisfaction and follow up regarding complaints
  • Manage communication between patients and aides
  • Document and oversee patient activity
  • Collaborate with colleagues in weekly coordination meetings
  • Follow up on contracts and communicate scheduling updates
  • Ensure proper follow-up on interdepartmental correspondence
  • Counsel PAs on incidents beyond a coordinator’s scope of practice
  • Verify PA attendance through electronic emails
  • Complete performance logs on a daily, weekly, and monthly basis

Requirements

  • Fluent in English and Spanish 🗣️
  • 3+ years of relevant experience, preferably in healthcare or homecare services
  • 2+ years in a supervisory or management role in home care
  • Proficiency in Microsoft Office Suite and HHA exchange (preferred)
  • Understanding of regulations related to Medicare, Medicaid, and other insurances
  • Excellent communication skills with employees, patients, families, and community partners
  • Knowledge of office operations for home care agencies
  • Strong customer service skills
