Job Summary

We are looking for a dynamic Home Care Marketer with extensive network connections and proven relationships within the healthcare industry. Your role will focus on community engagement, referral development, and educating various stakeholders about our services. Candidates with experience as a Paraprofessional, DSP, Community Habilitation staff, or in DOE/District 75 settings are highly encouraged to apply, as this helps in understanding client needs and service coordination.

Key Responsibilities 📝

• Represent the agency at community networking events
• Develop and execute strategic marketing plans with weekly effectiveness reports
• Maintain ongoing relationships with LHCSAs, IRAs, and other referral sources
• Identify opportunities to transition clients to CHHA services
• Educate caregivers, coordinators, and families about CHHA services and eligibility
• Foster strong referral relationships to ensure timely patient care
• Coordinate with B2B teams for smooth patient handoffs
• Maintain professional attire—jeans and sneakers are not permitted

Qualifications & Skills 🎯

Education

• High school diploma required
• College experience preferred, but equivalent work experience will be considered

Experience

• At least 2 years in marketing, business development, or referral coordination within healthcare or home care
• Solid understanding of LHCSA and CHHA operations and eligibility

Skills

• Excellent relationship-building and networking skills
• Strong communication and organizational abilities
• Proficiency in Microsoft Office Suite and CRM software
• Ability to collaborate across teams

Other Requirements

• Valid driver’s license
• Ability to travel within the service area as needed
