TEAM NY coordinates essential home care services through three Licensed Home Care Service Agencies (LHCSAs): ABI Healthcare, ATD Home Health Agency, and Life Improvement Homecare. Role Summary

We are looking for a passionate, organized, patient, and articulate Care Coordinator to join our team. You will be assigned to one of our three agencies where you will be a pivotal part of our operations. This role is the heart of our service delivery, acting as the primary point of contact for patients, caregivers, and families. What You’ll Do

  • Coordinate Care: Match seniors with caregivers and manage schedules to ensure seamless care delivery. Be the problem solver when issues arise.
  • Build Connections: Demonstrate empathy and patience, fostering meaningful relationships to enhance seniors' wellbeing.
  • Deliver Excellent Service: Respond quickly to questions and ensure families feel supported and valued.
  • Shape Strategy: Collect data and identify trends to support informed decision-making.
  • Maintain Business Flow: Work collaboratively with insurance partners to keep operations smooth and efficient.
  • Ensure Compliance: Safeguard patient, caregiver, and company data, train staff on electronic visit verification (EVV), and adhere to NY State regulations.

What You’ll Bring

  • Experience: Prior customer service experience and a genuine love for helping people.
  • Education: High school diploma or GED required.
  • Language Skills: Proficiency in English and Mandarin and/or Cantonese; additional Chinese dialects are a plus.
  • Technical Skills: Comfortable using computers and quick to learn new software tools.
  • Problem-Solving: Ability to resolve issues promptly, viewing challenges as opportunities.
  • Organizational Skills: Excellent time management and multitasking abilities.
  • Cultural Sensitivity: Deep respect and understanding of the diverse communities we serve.

Location & Details

Location: Flushing, NY (approximately 30-45 minutes from Manhattan). In-office work five days a week.

Job Type: Full-time

Pay Range: $20.00 - $23.00 per hour
