Community Health Worker
Inova Cares Clinic for Families in Dumfries is seeking a bilingual and committed Community Health Worker to provide compassionate care and resource navigation for our patients. This is a full-time position with a schedule of Monday to Friday, 8:30 a.m. to 5:00 p.m., including some evening and weekend hours.

Key Responsibilities
- Collaborate with community health, case management, and social services teams to coordinate short-term care and connect clients to resources
- Operate in both clinical and community settings, including clients' homes
- Serve as a liaison to facilitate access to health and social services, enhancing service quality and cultural competence
- Build individual and community capacity through outreach, education, counseling, social support, navigation, and advocacy
- Conduct community outreach activities such as home visits, assessments, health screenings, and care coordination
- Communicate the program’s purpose and impact, advocating for health equity and addressing socio-economic barriers
- Support clients in understanding and managing care plans, providing culturally appropriate health education
- Assist clients in accessing health services, overcoming barriers, and understanding emergency and community resources
- Motivate clients to actively participate in their health and well-being
- Track, report, and analyze community health data and program benefits
- Participate in quality improvement initiatives, community events, and departmental activities
- Maintain detailed electronic client records, including notes, assessments, and activity logs
- Coordinate referrals to health programs and follow-up services
- Represent the department in outreach activities and community events
- Some travel within Northern VA is required
Minimum Qualifications
Education: High School diploma or GED
Preferred Qualifications
- Background in community or public health
- Bilingual skills in Spanish
