Position: Home Care Client Services Coordinator

Location & Schedule

📍 Brooklyn, NY
🕒 Full-time, Monday–Friday, 9 am – 5 pm (onsite)

Position Summary

The Home Care Client Services Coordinator manages caregiver placement and visit confirmations for our patients. Responsibilities include developing and maintaining staff schedules, verifying attendance, and facilitating communication between caregivers, patients, and their families, all while adhering to True Care’s policies. The role also involves case management for an assigned caseload aimed at delivering top-quality service. 📝

Key Responsibilities

  • 🔑 Strategically coordinate care placements using HHAeXchange
  • 🩺 Maintain a caseload of 90-115 patients, ensuring excellent home care services
  • 🎓 Participate in departmental training sessions
  • 🤝 Assist the Team Lead with staffing coverage and challenges
  • 🗨️ Communicate with staff about scheduling, attendance, and larger operational issues
  • ✔️ Verify daily caregiver attendance and manage staffing to accommodate no-shows or call-outs based on patient needs
  • 📞 Handle incoming calls, emails, and faxes regarding reports and status updates, liaising with physicians and contracts
  • 💬 Coordinate appointment scheduling and communicate changes with patients and families
  • 📅 Maintain and document caregiver schedules and service records in HHAeXchange
  • 🔍 Verify billed services against requests for accuracy
  • 🙌 Ensure patient confidentiality, privacy, and dignity at all times
  • 📌 Participate in case conferences, training, caregiver exit interviews, and orientations
  • 🤗 Provide supportive communication to patients, families, physicians, and team members

Required Qualifications

  • 🎓 High School diploma or GED
  • 💻 Knowledge of HHAeXchange (highly preferred)
  • 🗣️ Bilingual fluency in Spanish
  • 🤝 Strong interpersonal and communication skills
  • 😊 Ability to handle sensitive situations with empathy and patience, especially with elderly clients
  • ⚡ Ability to multitask efficiently in a fast-paced environment
  • 🕴️ Professional demeanor when addressing issues
