PRINE Health is seeking a Bilingual (English/Spanish) Care Manager to join our dynamic care management team, dedicated to improving patient outcomes through comprehensive and coordinated care. 🌟

This full-time position, scheduled from Monday to Friday, 9am - 5pm, requires an enthusiastic, detail-oriented, and patient-focused individual eager to learn and grow within a supportive healthcare environment.

Role Overview

The Care Manager will develop and follow through on comprehensive care plans, maintaining monthly contact with patients. Educating patients on Chronic Kidney Disease and available treatment options is central to our mission — to coordinate care, empower patients, and promote healthier lifestyles.

Key Responsibilities

• Conduct patient assessments: Develop personalized care plans, set health goals, and document progress in electronic health records (EHR).
• Care coordination: Act as the main contact point to liaise with primary care physicians, specialists, and external healthcare providers.
• Patient education and counseling: Guide patients on managing chronic conditions, medication, and lifestyle changes.
• Patient engagement: Perform monthly check-ins, answer health questions, and provide 24/7 access to the care team.
• Transitions of care: Support seamless movement between healthcare settings to reduce readmissions.
• Record keeping: Maintain accurate health information, manage referrals, and document services in EHR systems.
• Remote monitoring: Use telehealth tools and digital communication to monitor and support patient health remotely.

Skills & Qualifications

• Strong clinical knowledge in managing chronic conditions
• Excellent communication, patient education, and motivational interviewing skills
• Proficiency with EHR and care coordination software
• Experience in inpatient and outpatient care coordination
• Ability to collaborate effectively with care teams and administrative staff
• Knowledge of healthcare policies related to chronic disease management
• Bilingual in English and Spanish (Required)

Preferred Experience & Certifications

• 1+ year in Medical Terminology and Chronic Care Management (Required)
• Health Coach/CCP certification (Preferred)
• Clinical Care Management experience with chronic diseases like Diabetes and Hypertension (Highly preferred)

Compensation & Benefits

Salary: $25.00 - $27.00 per hour

Additional Requirements

• Minimum 1 year RN or Care Management experience (Preferred)
• Certified Medical Assistant (Preferred)
