Responsibilities

• Member Needs Assessment: Conduct interviews—either remotely or in person—to identify individual needs and tailor personalized support.
• Advocacy: Connect members with vital services, guiding them through social, medical, and community programs.
• Progress Tracking: Monitor and document members’ progress, following up to address changing needs.
• Member Care & Reporting: Collect and analyze care data, preparing reports to evaluate program effectiveness.
• Collaboration: Partner with community organizations, healthcare providers, and social services to deliver comprehensive care.
• Assist individuals in understanding and navigating the healthcare system. 🏥
• Conduct in-home visits for members who are unreachable remotely, reviewing their needs and care access.
• Meet with members to assess housing conditions and identify care gaps.

Qualifications

• Experience in care management, social determinants of health (SDOH), or healthcare advocacy.
• Excellent communication and interpersonal skills, with the ability to build trust with diverse community members. 🤝
• Strong organizational and time management skills to handle multiple cases effectively.
• Ability to analyze data and generate reports.
• Knowledge of community-based services and social programs.
• Passion for supporting vulnerable populations aligned with the values of LOVE, LEARN, GROW.
• Effective outreach skills in Care Management.

Requirements

• Hold an active Community Health Worker (CHW) certification in New Jersey or possess relevant community outreach experience, with willingness to complete training upon hire.
• Bilingual in English and Spanish (spoken and written) is required. 🗣️
• Genuine interest in working with Medicare/DSNP/Medicaid populations and building supportive relationships.
• Excellent customer service and communication skills.
• Ability to clearly explain complex policies.
• Proven ability to manage multiple priorities in a fast-paced environment.
• Conflict resolution and people management skills.
• Reliable transportation and a valid driver’s license. 🚗
• Strong verbal and written communication skills to effectively engage community members.

Compensation Range: $22.22 – $25.51 per hour
