Program Overview

The Community Supportive Housing Program offers permanent supportive housing to formerly homeless individuals and families through scattered-site apartments utilizing the Housing First approach. Our goal is to remove barriers to housing for highly vulnerable and chronically homeless populations, supporting them through coordination of care, life skills development, and tenancy support.

Referrals for behavioral health services, including assertive community treatment, are integrated into the support provided. As a Case Manager, you'll manage a caseload of about 20 residents, conducting meetings at least twice per month to ensure their needs are met.

🔑 Essential Duties & Responsibilities

  • Collaborate trauma-informed with the Program Director and multidisciplinary team to offer personalized support that helps residents maintain housing.
  • Build rapport with clients by assessing ongoing needs and making appropriate service referrals.
  • Provide diverse client services such as:
    • Resource and referral assistance
    • Life skills training
    • Health maintenance and medication monitoring
    • Transport to appointments
  • Regularly update client charts including documentation of communication, entered into the Homeless Management Information System (HMIS).
  • Coordinate with apartment managers and landlords to ensure housing stability and negotiate lease agreements annually.
  • Complete annual certification documents for the County.
  • Conduct home inspections at least once per month to ensure property stability.
  • Maintain communication with Montgomery County, landlords, and property management for record-keeping and compliance.
  • Participate in county meetings, including monthly teaming and Housing Prioritization meetings.
  • Attend weekly supervision and staff gatherings.
  • Engage in Interfaith Works meetings: Monthly Case Management & Vocational sessions and other activities.
  • Perform other duties as assigned to support program goals.

🎓 Minimum Qualifications (Knowledge, Skills, and Abilities)

  • Bachelor’s degree in social services or related field; Master’s preferred in Social Work, Counseling, or Psychology.
  • At least two years of direct service experience working with homeless individuals with disabilities, including mental illness, substance use disorders, medical conditions, or cognitive impairments.
  • Knowledge of trauma-informed care principles and the Housing First model.
  • Experience with crisis intervention, including assessing and responding to suicidal or homicidal ideation.
  • Strong ability to handle multiple responsibilities efficiently.
  • Capability to work independently and collaborate effectively with teams across programs.
  • Excellent written and verbal communication skills.
  • Proficiency in Microsoft Windows and Office 365 (Outlook, Word, Excel, PowerPoint).
  • A clean driving record is required, as client transportation may be necessary.
  • Bilingual English/Spanish skills are preferred.

This role requires regular verbal communication and hearing ability. The employee frequently uses their hands and fingers to handle objects, tools, or controls. Occasional physical activities include standing, walking, sitting, reaching with hands/arms, and lifting or moving up to 25 pounds.
