Overview
As a Care Design New York Care Manager, your role is to assist people with intellectual and/or developmental disabilities in defining and achieving their long-term and short-term objectives by creating, executing, and monitoring their person-centered Life Plans. These plans empower our members to live their best lives, leveraging your exceptional relational and organizational skills. We welcome individuals from diverse backgrounds, all receiving top-tier training to excel in this rewarding profession.
This dynamic position operates in a hybrid environment, which includes:
- Required visits to members' homes and their families
- Periodic trainings and meetings in the office
- Work-from-home flexibility
Supporting members outside of New York City requires a driver’s license, reliable transportation, and appropriate insurance.

Responsibilities
- Advocate for members to ensure informed decision-making, consent, and guardianship.
- Coordinate and lead meetings and assessments with members and their interdisciplinary teams to develop comprehensive, person-centered Life Plans aligned with individual needs and preferences.
- Implement, update, and monitor Life Plans, facilitating reviews and approval processes.
- Integrate support services such as medical, behavioral, social, habilitation, dental, psychosocial, community-based, and long-term facility services.
- Maintain ongoing communication with key individuals in members’ lives.
- Ensure timely documentation submission, including Life Plans and progress notes.
- Assist members with benefits like Social Security, SSI, Medicaid, Medicare, and Food Stamps.
- Support problem resolution related to housing, utilities, legal matters, and safety.
- Report abuse or neglect immediately when observed or reported.

Qualifications
To qualify, Care Managers serving individuals with I/DD should meet the following criteria:
- A Bachelor’s degree with 2+ years of relevant experience, OR
- A License as a Registered Nurse with 2+ years of relevant experience, which can include a variety of employment experiences beyond case management, OR
- A Master’s degree with 1+ year of relevant experience.

Compensation
Starting at: $27.75 / hour
Maximum up to: $33.49 / hour
