Position Overview
As a Care Design New York Care Manager, you will assist individuals with intellectual and/or developmental disabilities in identifying and reaching their short-term and long-term goals. This is achieved by developing, implementing, and monitoring person-centered Life Plans that promote living life to the fullest. Your relational and organizational skills will be essential in providing exceptional service to our members.
Work Environment & Requirements
This dynamic role is performed in a hybrid environment that includes:
- Required visits with members and their families at home 🏡
- Periodic trainings and meetings at our office 🏢
- Remote work from home 💻
For members outside New York City, reliable transportation, a driver’s license, and appropriate insurance are necessary.

Key Responsibilities
- Advocate for members to ensure informed decision-making, consent, and guardianship 🗣️
- Coordinate meetings and assessments with members and interdisciplinary teams 🤝
- Implement, update, and monitor Life Plans and facilitate reviews and approvals 🔄
- Integrate support services, including medical, behavioral, social, habilitation, and community-based supports 🏥🎉
- Maintain ongoing contact with key people in members’ lives 📞
- Ensure timely documentation submission (Life Plans, progress notes, etc.) 📝
- Assist members with maintaining benefits such as Social Security, Medicaid, Medicare, and Food Stamps 💰
- Help resolve living and safety issues related to housing, utilities, and legal matters 🏠⚖️
- Report abuse or neglect immediately 🚨

Qualifications
Candidates serving individuals with I/DD must meet the following criteria:
- Bachelor’s degree with 2+ years of relevant experience, OR
- Registered Nurse license with 2+ years of relevant experience, OR
- Master’s degree with 1+ year of relevant experience

Compensation
The hourly rate for this position ranges from $27.75 to $28.41 per hour. 💵
