About the Role

As a Care Manager at Care Design New York, you will help members identify and achieve their short-term and long-term goals through developing, implementing, and monitoring person-centered Life Plans. Your relational and organizational skills will be vital in empowering our members to live their best lives. Our comprehensive training ensures all Care Managers are well-prepared to deliver the highest standard of care, regardless of background.

Work Environment & Requirements

  • Hybrid setup: visits with members and families, office trainings and meetings, and remote work 🏠
  • For members outside New York City: valid driver’s license, reliable transportation, and appropriate insurance are required 🚗

Key Responsibilities

  • Advocate for members to promote informed decision-making, consent, and guardianship 🗣️
  • Schedule and lead meetings with members and interdisciplinary teams to develop comprehensive, person-centered Life Plans 🎯
  • Implement, update, and monitor Life Plans; facilitate reviews and approvals 🔄
  • Coordinate supports and services across medical, behavioral, social, habilitation, and community-based resources 💼
  • Maintain ongoing contact with key individuals in members’ lives 📞
  • Ensure timely documentation, including Life Plans and progress notes 📝
  • Assist members with benefits management such as Social Security, Medicaid, Medicare, and Food Stamps 💳
  • Support members in resolving living challenges related to housing, utilities, safety, and legal issues 🏡⚖️
  • Report abuse or neglect immediately 🆘
  • Additional details provided during interviews

Qualifications & Requirements

  • Bachelor’s degree + 2+ years relevant experience, OR
  • License as a Registered Nurse + 2+ years relevant experience, OR
  • Master’s degree + 1+ year relevant experience 🎓

Strong communication, organizational, and interpersonal skills are essential. Bilingual Spanish candidates are preferred.

Compensation 🔢

Starting at $23.75 / hour, with opportunities to earn up to $28.07 / hour.
