Charles B. Wang Community Health Center Is Hiring A Care Management Specialist Bilingual

Join the Charles B. Wang Community Health Center as a Care Management Specialist (Bilingual in English/Chinese). We are looking for a dedicated professional to support mental health services in Flushing, NY.

Responsibilities:

• Coordinate mental health treatment for pediatric and/or adult patients and their families
• Assist mental health clinicians in providing treatment and crisis intervention
• Provide ongoing support and case management to patients
• Partner with providers to implement depressive collaborative care management
• Conduct screening for depression and other mental health issues
• Perform immediate screening for incoming referrals
• Request and follow up on psychiatric medication pre-authorization
• Follow up on no-show patients, at-risk individuals, ER referrals, and hospitalizations
• Manage the Mental Health Direct Line
• Support mental health providers in designing and facilitating workshops and therapy groups
• Maintain care management-related patient databases
• Prepare reports and documentation related to services
• Serve as liaison with community agencies and schools
• Monitor and manage the mental health patient portal
• Document all service contacts in electronic medical records
• Attend relevant treatment team meetings
• Perform other duties as assigned

Minimum Qualifications:

• Bachelor's degree in social services, psychology, or related field
• Availability to work weekends and early evenings
• Strong interpersonal and communication skills
• Ability to work independently
• Bilingual in English and Chinese (Mandarin and/or Cantonese) is required
• Proficient in computer software applications

Compensation & Benefits:

• Salary: $40,000.00 – $45,000.00 per year
