💼 Job Responsibilities

• Provide psychosocial support counseling for hospice and palliative care patients and families in the community homecare setting, referring to hospice as needed
• Serve as part of the Hospice and Palliative Care Interdisciplinary Team (IDT), offering counseling and case management services
• Complete 20-25 patient visits weekly, documenting all activities in electronic medical records
• Develop social work plans of care, including goals, interventions, and community referrals
• Create and update palliative social work care plans tailored to patient and family needs
• Ensure compliance with documentation policies and procedures
• Set specific goals and outcomes with targeted end dates for interventions
• Coordinate transitional planning and community linkages, including hospice eligibility screening and discharge planning activities

🎓 Educational & Experience Requirements

• Graduate from an accredited School of Social Work
• Minimum of 3 years’ experience in healthcare social work
• Hospice or community health experience preferred
• Must hold current LMSW certification
• Must have a reliable car and a valid driver’s license
