🔑 Key Responsibilities

• Communicate with patients and families to assess care needs and provide ongoing support 🗣️
• Develop, coordinate, and manage individualized patient care plans 📋
• Schedule and coordinate appointments and services with healthcare providers 📅
• Educate patients and families on medications, conditions, and care instructions 💊
• Monitor patient progress and maintain accurate documentation 📄

🎯 Qualifications

• Minimum 3 years experience in care coordination, healthcare administration, or clinical practice 🏥
• Bilingual in English and Bengali required 🗣️
• Strong communication, organizational, and multitasking skills 🧠
• Proficiency in Microsoft Office and general computer systems 💻
• Ability to work in a fast-paced healthcare environment ⚡
