About the Opportunity: Bilingual Family Nurse Practitioner 🌟

Pair Team is hiring a Bilingual Family Nurse Practitioner to support the launch of our Medicare ACCESS program across multiple states. In this role, you will provide virtual care to Medicare patients with complex medical, behavioral, and social needs.

Key Responsibilities

  • Provide virtual clinical assessments and care for Medicare patients across ACCESS markets
  • Support initial assessments, care plan development, and clinical oversight
  • Manage chronic conditions, acute concerns, medication questions, and routine follow-ups via telemedicine
  • Collaborate with RNs, Community Health Workers, and leadership to close care gaps and ensure timely follow-up
  • Offer clinical guidance during case reviews, escalations, and complex patient cases
  • Support patients with conditions common in Medicare populations, including hypertension, diabetes, CKD, cardiovascular disease, chronic pain, depression, and anxiety
  • Build trusted relationships with patients who may have limited access to continuous care
  • Work with operations and product teams to optimize care workflows, documentation, and engagement
  • Maintain accurate, timely, and compliant clinical documentation

What You'll Need to Succeed 🎯

  • Active Nurse Practitioner licenses in at least two states among California, Nevada, Pennsylvania, New York, Maryland, Georgia, West Virginia, Texas, Florida, or North Carolina, with willingness to obtain additional licenses as needed
  • Board certification as a Family Nurse Practitioner or Adult-Gerontology Nurse Practitioner
  • Fluency in both English and Spanish
  • At least 2 years of clinical experience as a Nurse Practitioner
  • Experience caring for Medicare, geriatric, or complex adult populations
  • Comfort managing chronic conditions like hypertension, diabetes, CKD, etc., via telemedicine
  • Familiarity with digital platforms and telehealth technology
  • Ability to work across EST and PST time zones
  • Experience within a multidisciplinary care model
  • Access to a private, HIPAA-compliant remote workspace with reliable internet
