Key Responsibilities 📝

• Conduct individual and group psychotherapy sessions in skilled nursing or subacute facilities
• Assess, diagnose, and treat emotional and behavioral health conditions such as depression, anxiety, adjustment disorders, and cognitive decline
• Develop and implement evidence-based, personalized treatment plans
• Collaborate with facility staff, interdisciplinary teams, and family members to ensure optimal care
• Maintain accurate clinical documentation in compliance with regulations using our user-friendly EHR system
• Monitor patient progress and adapt treatment strategies as needed

Requirements ✅

• Active, unrestricted LCSW license in the state of application
• Proficiency in both English and Spanish, oral and written
• Basic technological skills, including familiarity with EHR systems
• Strong organizational and documentation skills ensuring compliance
• Ability to provide in-person services within 45 minutes at designated facilities
• Physical capacity to walk, stand, and move within the facility throughout the day
• Full vaccination and booster status may be required in certain facilities

Preferred Qualifications 🌟

• Prior clinical experience in subacute, long-term, or geriatric healthcare settings
• Experience treating anxiety, depression, and adjustment disorders
• Psychology, addiction, or related specialization a plus
• Excellent time management and organizational skills
