Key Responsibilities 📝
- Conduct individual and group psychotherapy sessions in skilled nursing or subacute facilities
- Assess, diagnose, and treat emotional and behavioral health conditions such as depression, anxiety, adjustment disorders, and cognitive decline
- Develop and implement evidence-based, personalized treatment plans
- Collaborate with facility staff, interdisciplinary teams, and family members to ensure optimal care
- Maintain accurate clinical documentation in compliance with regulations using our user-friendly EHR system
- Monitor patient progress and adapt treatment strategies as needed
Requirements ✅
- Active, unrestricted LCSW license in the state of application
- Proficiency in both English and Spanish, oral and written
- Basic technological skills, including familiarity with EHR systems
- Strong organizational and documentation skills ensuring compliance
- Ability to provide in-person services within 45 minutes at designated facilities
- Physical capacity to walk, stand, and move within the facility throughout the day
- Full vaccination and booster status may be required in certain facilities
Preferred Qualifications 🌟
- Prior clinical experience in subacute, long-term, or geriatric healthcare settings
- Experience treating anxiety, depression, and adjustment disorders
- Psychology, addiction, or related specialization a plus
- Excellent time management and organizational skills

Compensation
Annual Salary Range: $56,000 - $85,000
