The Bilingual Case Manager plays a vital role in delivering counseling, support, and educational interventions to youth involved in the Power Project, ensuring comprehensive case management and community engagement. Job Responsibilities 🎯
- Ensure health and safety risks in families are identified, controlled, or eliminated, and address clinical needs effectively.
- Manage and mitigate risk within assigned areas, escalating issues that require additional support.
- Work effectively with culturally and economically diverse populations.
- Assist in providing safe, secure, and structured onsite activities for youth, emphasizing runaway prevention and substance use reduction.
- Meet with families in their communities to support stability goals, providing case management, advocacy, and crisis intervention.
- Help youth and families access services, community supports, advocacy groups, and educational opportunities.
- Teach parents strategies for working effectively with their children during and after the program.
- Facilitate parent support groups and collaborate with team members including the Substance Abuse Counselor and Peer Mentor.
- Conduct assessments, participate in treatment planning, and process referrals efficiently.
- Monitor overall service delivery and ensure completion of comprehensive assessments and discharge plans for each family.
- Support the development of independent living skills as needed.
- Act as a liaison to community networks such as schools, recreational and after-school programs, mental health providers, and medical services.
Job Requirements ✔️
- Bachelor’s degree in social work or a related human services field.
- At least 1 year of experience working with youth/adolescents with complex psychological, emotional, and behavioral needs, maintaining therapeutic relationships.
- Flexibility to work hours after 6 pm as needed; training schedule will start at 9 am.
- Strong knowledge of entitlements (e.g., SSI, Medicaid), educational placements, housing systems, family court, and juvenile justice system, especially within the Bronx community.
- Bilingual in Spanish and English is required.
- Excellent communication, assessment, and written skills.
- Experience in conducting group and individual counseling.
- Proficiency in conducting meetings and training sessions with families.
- Proven skills in computer use and data entry.
- Ability to work collaboratively within a multi-service agency and communicate effectively.
- Valid New York State driver’s license preferred; ability to use public transportation across NYC required.
