🔍 Summary
The bilingual Family Support Case Worker will develop individual child case plans, track progress, and maintain comprehensive case files. This includes providing information, education, referrals, outreach, advocacy, and support to facilitate the post-release home placement of children. The role involves building a network of contacts and referral resources to support sponsors and children, assessing safety and appropriate placement for children within the Juntos home study program, with the ultimate goal of promoting family unification, family preservation, kinship care, and overall child well-being.
📝 Duties and Responsibilities
- Provide information, education, referrals, outreach, advocacy, and support to ensure children's needs are met.
- Continuously assess safety and suitability of placements using child welfare principles and best practices, including conducting calls, interviews, and home visits.
- Develop culturally sensitive and individualized case plans considering language and unique circumstances.
- Maintain accurate case files by updating databases, submitting reports, and managing physical case files.
- Build a network of local legal resources, food and clothing banks, mental health services, and care providers to support youth.
- Track progress via online databases and coordinate communication with key stakeholders.
- Participate in regular training sessions with staff, supervisors, and headquarters.
- Ensure confidentiality and adhere to professional and ethical standards outlined in the employee handbook.
- Maintain professionalism, respect, and proper dress code at all times.
- Meet or exceed productivity expectations consistently.
- Perform remote duties from home as required.
- Perform other duties as assigned by the supervisor.
🎓 Requirements
- Bachelor's degree in Social Work (BSW) or related field such as education, psychology, sociology, or behavioral sciences.
- Fluency in English and Spanish
