We are seeking a compassionate, detail-oriented telehealth registered nurse to support patients with rare or complex diseases through education, coordination, and adherence support.

Purpose

The Bilingual Care Navigator (CN) is a telehealth registered nurse dedicated to providing empathetic and clinically informed support to patients. This role combines clinical expertise, patient education, regulatory knowledge, and meticulous documentation to ensure timely therapy initiation and ongoing health management. Acting as a vital link among patients, healthcare providers, infusion sites, and internal/external teams, the CN promotes a seamless patient journey and optimal health outcomes.

Key Responsibilities

Clinical Education & Adherence Support 💊

  • Apply motivational interviewing techniques and assessment tools to evaluate patient knowledge, ability, and readiness to learn.
  • Use approved training materials to educate patients on disease management and infusion logistics.
  • Guide patients through therapy initiation, medication administration, dosing, and side effect management.
  • Design personalized adherence plans with scheduled follow-ups to build trust and empower self-management.
  • Assist patients with affordability options, including copay assistance and financial support programs.

Program Operations, Compliance & Quality ✅

  • Ensure program activities meet contract deliverables, KPIs, and regulatory standards.
  • Support patient onboarding and coordinate with healthcare providers and care sites as needed.
  • Build strong relationships with patients, caregivers, providers, pharmacies, payers, and industry representatives.
  • Identify and escalate risks, safety issues, adverse events, and product complaints per policy.
  • Participate in team discussions to improve program efficiency, patient experience, and clinical workflows.
  • Maintain confidentiality and full compliance with HIPAA and privacy laws.
  • Demonstrate positivity, accountability, and adherence to nursing standards.

Required Qualifications 🎯

  • Fluent in Spanish to serve Spanish-speaking regions.
  • Active, unrestricted RN license in good standing; Associate or Bachelor’s Degree in Nursing.
  • Minimum of 2–3 years of clinical nursing experience; telehealth experience in adherence support, patient case management, or pharmaceutical/biotech sectors highly preferred.
  • Strong understanding of third-party reimbursement and affordability coverage.
  • Excellent verbal and written communication skills, capable of explaining complex concepts virtually.
  • Ability to independently manage caseloads, prioritize tasks, and meet program goals.
  • Proficiency in Microsoft Word, Excel, and Outlook, with strong data-entry skills.
  • Demonstrated ability to exercise independent judgment and maintain detailed documentation.

Preferred Qualifications 🌟

  • Clinical experience in infusion therapy and/or rheumatology.
  • Experience with adherence outreach utilizing motivational interviewing or behavioral coaching.
  • Previous engagement with multiple healthcare stakeholders including offices, infusion centers, and pharmacies.
  • Proven ability to resolve complex issues with urgency and empathy.
  • Ability to adapt swiftly to evolving healthcare environments and diverse systems.
  • Familiarity with adverse event and product complaint reporting processes.

Work Environment 🏢🏠

RareMed offers a hybrid work model combining remote and in-office work. Onsite requirements will vary based on operational needs, meetings, or client visits, with advance notice provided whenever possible. Employees will use standard office equipment and must have reliable internet access for remote work. The role requires setting up a dedicated home office space that ensures privacy and productivity. Regular reporting to the office may be required as needed.
