Position Details

Salary: $55,702.00

Work Schedule: Monday - Friday, 8:30 AM - 5:00 PM

Job Summary

As a Bilingual Blended Case Management Supervisor, you will lead and support our case management team, supervise Targeted Case Managers, provide guidance on service delivery, ensure compliance with program standards, and oversee care coordination for adults in the community. Your leadership will foster a collaborative environment and ensure the delivery of high-quality, client-centered services.

Key Responsibilities

• Ensure implementation of DBH Practice Guidelines within TCM duties.
• Provide daily on-site supervision to Teams of Blended Case Managers, facilitating team meetings to discuss daily contacts and address any issues.
• Conduct weekly individual supervision sessions with staff, documenting reviews and utilizing updated Utilization Review tools.
• Monitor daily the 30-day contact board to ensure contacts are completed timely to prevent infractions.
• Ensure all Pay 4 Performance criteria are met, including timely, legible documentation following the DAP format.
• Monitor staff productivity through reports provided by the TCM director.

Requirements

• Master’s degree in social work, psychology, rehabilitation, activity therapies, counseling, or education
• At least three years of mental health direct care experience, with supervisory experience preferred
• Knowledge of and ability to implement Blended Case Management Standards of Care
• On-call responsibility & completion of BHTEN 10-week case management orientation
• Valid driver’s license and ability to operate program vehicles
• Bilingual proficiency in English and Spanish with strong reading and writing skills
• Satisfactory driving record and current driver’s license
