Salary: $55,702.00 per year
Schedule: Monday - Friday, 8:30 AM - 5:00 PM
Job Summary
We are seeking a Bilingual Blended Case Management Supervisor to lead and support our case management team. In this role, you will supervise Targeted Case Managers, provide guidance on service delivery, ensure compliance with program standards, and oversee care coordination for adults in the community. Key Responsibilities
- Ensure the implementation of DBH Practice Guidelines within the daily operations of TCM duties.
- Provide on-site supervision of teams of Blended Case Managers, including conducting team meetings to discuss daily contacts and support problem-solving efforts.
- Conduct weekly individual supervision sessions with staff, document these reviews, and perform chart reviews focusing on PGP coverage since the last CBH review.
- Monitor the 30-day contact board daily, ensuring contacts are completed timely to prevent infractions.
- Ensure all Pay for Performance criteria are met, including timely, legible documentation following DAP format.
- Supervise staff productivity using reports provided through the TCM director.

Requirements
Qualifications & Skills
- Master's degree in social work, psychology, rehabilitation, activity therapies, counseling, or education
- At least three years of mental health direct care experience, including supervisory experience preferred
- Knowledge of and ability to apply Blended Case Management Standards of Care
- On-call responsibility & completion of BHTEN 10-week orientation in case management
- Valid driver's license with a satisfactory driving record and ability to operate program vehicles
- Proficiency in both English and Spanish, with strong reading and writing skills
