Position Overview

Work Schedule & Flexibility

This role allows associates to work remotely full-time, with the exception of required in-person training sessions, ensuring maximum flexibility and autonomy.

• Work Shift: Monday – Friday, 8:00 am to 5:00 pm CST or EST ⏰
• On-Call: Rotating schedule

Responsibilities 🩺

• Provide primary and urgent healthcare via telephone and tele-video to patients in home and community settings.
• Develop and implement comprehensive care plans for adults with chronic and complex conditions, including mental health co-morbidities.
• Conduct patient history taking, physical exams, diagnostics, and develop treatment strategies aligned with patient goals.
• Identify care gaps and work to close them to improve patient outcomes.
• Address physical and psychosocial patient needs with support from an interdisciplinary team.
• Educate patients and families on medication management, side effects, illness progression, nutrition, adherence, and crisis prevention.
• Coordinate with clinical team members, physicians, and providers to ensure consistent, compliant care.
• Maintain accurate medical records in accordance with regulatory standards and company policies.
• Participate in ongoing education and training required by state and certifying bodies. Prescribe medications as permitted by state law.

Minimum Requirements 📋

• MS in Nursing
• Active, national NP certification
• Valid, current, and unrestricted Family or Adult NP license in New York
• Valid RN Compact license
• Minimum of 2 years managing complex care cases
• Experience with Electronic Medical Records (EMR)
• Bilingual in English and Mandarin

Preferred Skills & Qualifications 🌟

• DEA registration or eligibility
• Active Medicaid licensure in New York
• Experience managing complex cases involving developmental disabilities and chronic illnesses

Location & Compensation 💰

Location: New York

Salary Range: $115,184 – $198,144, based on experience, education, and skill level
