Position: Bilingual Advanced Practice Provider (English/Mandarin)

Role at: Care Bridge Bilingual Telehealth Nurse Practitioner

Sign-on Bonus: $5,000 💰

Location:

Virtual (Remote): This role allows associates to work remotely full-time, with only required in-person training sessions, providing maximum flexibility and autonomy.

Work Schedule:

• Monday – Friday, 8:00 am to 5:00 pm CST or EST
• Rotating on‑call shifts

Responsibilities

• Provide primary and urgent healthcare via telephone and tele-video to patients receiving home and community-based services through state Medicaid programs and partners.
• Develop and implement clinical care plans for adult patients with chronic and complex conditions, including co-morbid medical and mental health diagnoses.
• Perform patient history assessments, physical exams, diagnostics, and develop treatment plans aligned with patient goals and current health status.
• Identify and address gaps in care to optimize patient outcomes.
• Support the physical and psychosocial needs of patients and families with input from the interdisciplinary team.
• Educate patients and families on medication management, side effects, illness progression, diet, nutrition, adherence, and crisis prevention.
• Coordinate with other clinical team members, physicians, and healthcare providers to ensure timely, consistent, and compliant care delivery.
• Maintain accurate patient medical records following state regulations and company policies.
• Participate in ongoing continuing education as required by state and certifying bodies; prescribe medications where permitted.

Minimum Requirements

• Master of Science in Nursing (MS)
• Active, national Nurse Practitioner (NP) certification
• Valid, current, unrestricted Family or Adult NP license in New York
• Valid, current RN Compact license
• At least 2 years of experience managing complex care cases
• Proficiency with Electronic Medical Records (EMR)
• Bilingual proficiency in English and Mandarin 🈶️🈚️

Preferred Skills, Qualifications & Experience

• DEA registration or eligibility
• Active Medicaid licensure in New York
• Experience managing developmental disabilities and chronically ill patients

Location: New York, NY

Salary Range: $115,184 to $198,144 based on experience, education, and skills 💵
