🌟 Position Overview

Care Bridge Bilingual Telehealth Nurse Practitioner

Sign-On Bonus: $5,000 💰

📍 Location

Type: Virtual (Remote)

This role allows associates to work full-time remotely, with the exception of required in‑person training sessions, offering maximum flexibility and autonomy.

🕗 Work Schedule

• Monday – Friday
• 8:00 am to 5:00 pm CST or EST
• Rotating on‑call

🎯 Responsibilities

• Provide primary and urgent healthcare via telephone and tele-video to patients in home and community-based services through state Medicaid programs and partners.
• Develop and implement clinical care plans for adults with chronic and complex conditions, including co-morbid medical and mental health diagnoses.
• Perform patient history, physical exams, diagnostics, and develop treatment plans aligned with patient goals and current health conditions.
• Identify and address gaps in care to ensure comprehensive treatment.
• Support patients and families in physical and psychosocial needs with collaboration from the interdisciplinary team.
• Educate patients and families on medication management, side effects, illness progression, diet, nutrition, adherence, and crisis prevention.
• Coordinate with clinical team members, physicians, and other providers to deliver timely, consistent, and compliant care.
• Maintain accurate and compliant patient medical records per state regulations and company policy.
• Participate in ongoing continuing education and prescribe medications within legal scope.

🎓 Minimum Requirements

• Master's degree in Nursing (MSN)
• Active, national Nurse Practitioner (NP) certification
• Valid, current, active, and unrestricted Family or Adult NP license in New York
• Valid, current, active RN Compact license
• At least 2 years of experience managing complex care cases
• Experience working with Electronic Medical Records (EMR)
• Bilingual in English and Mandarin

✨ Preferred Skills, Qualifications, and Experience

• DEA registration or eligibility
• Active Medicaid licensure in New York
• Experience managing complex cases involving developmental disabilities and chronic illnesses

💼 Location and Compensation

Location: New York

Salary Range: $115,184 – $198,144 (based on experience, education, and skill level)
