Position: Bilingual Advanced Practice Provider (English/Mandarin)

Care Bridge Bilingual Telehealth Nurse Practitioner

💰 $5,000 sign-on bonus

Location:

Virtual (This role offers the flexibility to work remotely full-time, with only in-person training sessions required occasionally.)

Work Shift:

Monday – Friday, 8:00 am to 5:00 pm CST or EST, with rotating on-call responsibilities.

Responsibilities

• 🌟 Provide primary and urgent healthcare via telephone and tele-video modalities to patients receiving home and community-based services through state Medicaid programs and partners.
• 📝 Develop and implement clinical care plans for adult patients with chronic and complex conditions, including co-morbid medical and mental health diagnoses.
• 🩺 Perform patient history, physical exams, diagnostics, and develop treatment plans aligned with patient goals and current conditions.
• 🚶‍♂️ Identify and close gaps in care to improve health outcomes.
• 🤝 Address patient and family physical and psychosocial needs with support from an interdisciplinary team.
• 📚 Educate patients and families on medication use, side effects, illness progression, diet, nutrition, adherence, and crisis prevention.
• 🤝 Coordinate with clinical team members, physicians, and healthcare providers to ensure timely, consistent, and compliant care.
• 🗂 Maintain accurate medical records in accordance with state regulations and company policies.
• 🎓 Participate in continuing education and, where permitted, prescribe medications as authorized by state law.

Minimum Requirements

• 🎓 MS in Nursing
• ✅ Active, national Nurse Practitioner (NP) certification
• 📝 Valid, current, unrestricted Family or Adult NP license in New York
• 📄 Valid, current RN Compact license
• 🕒 Minimum of 2 years managing complex care cases
• 💻 Experience working with Electronic Medical Records (EMR)
• 🗣 Bilingual in English and Mandarin

Preferred Skills, Qualifications, and Experience

• 📝 DEA registration or eligibility
• 🏥 Active Medicaid licensure in New York
• 💡 Experience managing complex cases involving developmental disabilities and chronic illnesses

Location and Compensation

🌎 **Location:** New York

💰 **Salary Range:** $115,184 – $198,144 (dependent on experience, education, and skill level)
