Overview

Provides clinical leadership to promote adherence to a variety of quality, safety, and cost standards. Manages service delivery by collaborating with inter-professional and para-professional team members on individual cases or populations. Acts as a key resource in delivering clinical and operational guidance to teams and staff to achieve superior outcomes. Offers advanced nursing clinical care aligned with state, federal regulations, and national standards. Works under the supervision of the Clinical Director for the Nurse Practitioner Program.

What Will You Do? 🩺

• Manage and perform full scope of advanced nursing practice for targeted patient populations
• Evaluate patient responses to therapies and interventions
• Revise inter-professional care plans to meet quality outcomes
• Identify the need for new or revised clinical protocols
• Collaborate with physicians to develop and train on protocols
• Provide comprehensive advanced nursing care, including physical exams, history assessments, screenings, emergent interventions, pharmacological and non-pharmacological treatments, and preventative health maintenance
• Order treatments, DME, and referrals; discharge planning; patient counseling and education
• Develop treatment plans based on clinical findings
• Determine when further evaluation by specialists or emergency care is required
• Work collaboratively with patients, families, and healthcare teams for assessment and care planning
• Provide continuous care management across various settings
• Ensure services meet standards such as Patient-Centered Medical Home, HEDIS, and QARR quality metrics

Qualifications 📋

Licenses and Certifications

• Current RN license and registration in New York State
• Active Nurse Practitioner certification in adult health, family health, or gerontology (NY State required)
• Valid driver’s license (dependent on regional needs)
• Credentialed with VNS Health Medical Care at Home and relevant managed care plans
• NPI, Medicaid, and Medicare provider numbers preferred
• Collaborative practice agreement with a physician (NY State regulation preferred)
• Certification by ANCC or equivalent accrediting body (required to bill Medicare)
• Psychiatric Nurse Practitioners: Current PMHNP-BC certification (required)

Education 🎓

• Master’s Degree in Nursing or related graduate degree from an accredited nurse practitioner program (NY State registered) – required
• Current ANCC or AANP certification as adult, family, or geriatric nurse practitioner – required
• Psychiatric Nurse Practitioners: Master’s in psychiatric-mental health Nurse Practitioner – required
• PhD in psychiatric-mental health Nurse Practitioner – preferred

Work Experience 💼

• Minimum two years’ experience as a full-scope nurse practitioner (preferred)
• Clinical home care or managerial experience (preferred)
• Knowledge of HEDIS, QARR, ICD-10, CPT coding for reimbursement (required)
• Bilingual skills, as operational needs determine

Compensation 💰

Salary Range: USD $109,900.00 – USD $146,500.00 per year
