Your Responsibilities 📝

• Manage and provide full-scope advanced nursing practices for targeted patient populations 🩺
• Evaluate patient responses to therapies and adjust care plans accordingly 🔄
• Develop, revise, and implement clinical protocols collaborating with physicians 📝
• Deliver comprehensive nursing care, including physical exams, history-taking, screening, interventions, treatment ordering, and patient education 📋
• Establish and adjust treatment plans based on clinical findings, determining when further evaluations are needed 📈
• Coordinate with patients, families, and healthcare team members for assessment and care planning 👨‍👩‍👧‍👦
• Provide services compliant with Patient-Centered Medical Home standards, meaningful use, HEDIS, and QARR quality measures 🏥

Qualifications & Licensing ✅

Licenses and Certifications:

• Current NY State Registered Nurse license & registration 📝
• Licensed Nurse Practitioner in NY, with specialty in adult, family, or gerontology 🏥
• Valid driver’s license (may be required based on operational needs) 🚗
• Credentialed with VNS Health Medical Care at Home and managed care plans ✅
• NPI, Medicaid, and Medicare provider numbers (preferred) 💳
• Collaborative practice agreement with a physician (preferred) 🤝
• Certification by ANCC or equivalent (mandatory for billing Medicare) 🎖️

Specialized for Psychiatric Nurse Practitioners:

• Current PMHNP-BC certification 🧠

Educational Requirements 🎓

• Master's Degree in Nursing or equivalent from an accredited nurse practitioner program 📜
• Current ANCC or AANP certification as an adult, family, or geriatric nurse practitioner 🩺
• Psychiatric NPs: Master’s and optionally PhD in psychiatric-mental health (preferred) 🎓

Work Experience 💼

• Minimum two years as a practicing nurse practitioner (preferred) ✅
• Experience in home care or managerial roles (preferred) 🏠
• Knowledge of HEDIS and QARR quality measures, ICD-10, CPT coding required 📊
• Bilingual skills based on operational needs (required) 🗣️

Compensation 💸

Annual salary ranges from USD $109,900 to USD $146,500.
