Position Responsibilities

  • Conduct wellness assessments to identify needs and develop personalized wellness plans covering physical, social, cognitive, spiritual, and mental health domains
  • Coordinate with healthcare providers including primary care, mental health, and hospitals
  • Partner with housing staff to tailor programs meeting resident needs
  • Educate staff members on recognizing and communicating residents' health changes
  • Assist residents in completing health documentation including end-of-life planning and emergency contacts
  • Support onsite services and connect residents to off-site resources by follow-up and adherence monitoring
  • Develop relationships with payers serving seniors in the community
  • Implement communication systems between housing and providers for updates on residents’ health and behavior
  • Promote self-care through coaching and coordinate services that support personal goals
  • Refer residents to local service providers such as Visiting Nurses, rehabilitation, PACE, and memory programs
  • Lead and participate in care planning meetings
  • Assess residents with dementia and make appropriate referrals
  • Collaborate with Nurse Care Manager on evidence-based program planning and implementation
  • Coordinate safe discharges with hospitals and home care services
  • Assist with care transitions and family meetings as needed
  • Advocate for residents and serve as a liaison for families
  • Track metrics and outcomes, creating reports to measure success

Qualifications

  • 🎓 Masters in social work or human services with 1-2 years of experience in aging services OR Bachelors with 3-5 years of relevant experience
  • ❤️ Compassionate and motivated to work with seniors
  • 🗂️ Excellent organizational and interpersonal skills for managing multiple projects
  • 🤝 Professional, proactive, collaborative, and results-driven
  • 🌟 Positive attitude, strong communication, intuitive judgment
  • 🧠 Willing to learn, flexible, and capable of clear documentation in AASC Online software
  • 💻 Computer literacy with Windows, Word, PowerPoint, and Excel
  • 🚗 Ability to travel locally within the Boston area for site visits and meetings
  • 🧓 Certification in Dementia Care preferred
  • 🗣️ Fluency in Spanish is mandatory

Salary Range

$53,971.45 - $80,957.18
