Primary Responsibilities

  • Conduct comprehensive wellness assessments and develop personalized wellness plans covering physical, social, cognitive, spiritual, and mental health
  • Coordinate care with primary care providers, mental health professionals, and hospitals
  • Partner with housing staff to customize programming meeting resident needs
  • Educate staff on identifying and communicating residents’ health changes
  • Assist residents in completing health records, end-of-life planning, and emergency contacts
  • Follow up with at-risk residents to ensure adherence to wellness activities
  • Develop relationships with payers and ensure seamless communication between housing and healthcare providers
  • Promote self-care and coordinate supportive programs aligned with residents’ personal goals
  • Refer residents to local service providers such as Visiting Nurses, Rehabilitation Services, PACE, and Memory Programs
  • Lead or participate in care planning meetings onsite
  • Assess and refer residents with dementia appropriately
  • Collaborate with Nurse Care Managers on Evidence-Based Programs
  • Coordinate safe discharges with hospitals and case management teams
  • Support transitions to other levels of care, including family and team meetings
  • Advocate for residents and serve as a liaison for families
  • Track metrics and generate reports on program and resident outcomes

Qualifications

  • Master’s degree in social work or related human services field; 1-2 years of case management experience in aging services preferred
  • Or Bachelor’s degree with 3-5 years of relevant experience
  • Deep compassion for and desire to work with seniors
  • Strong organizational, interpersonal, and multitasking skills
  • Professional, proactive, collaborative, and results-driven attitude
  • Positive demeanor, excellent communication, sound judgment, and intuition
  • Willingness to learn and adapt to changing needs
  • Proficiency in documenting using AASC Online software
  • Computer skills: Windows, Word, PowerPoint, Excel
  • Availability for travel within Boston metro area
  • Certification in Dementia Care preferred
  • Fluency in Spanish is required 🗣️

Location: On-site at an affordable housing community in Chelsea

Salary Range: $53,971.45 - $80,957.18
