Job Title: Wellness Coordinator (R3 Program) 🎯

Overview

The Wellness Coordinator (WC) plays a pivotal role within the R3 program, actively engaging residents in wellness assessments, health education, and connecting them to essential services. This on-site position in Chelsea emphasizes collaboration with healthcare providers, staff, and residents, with a primary focus on Spanish-speaking seniors. Fluent Spanish language skills are required. 🗣️

Position Responsibilities

  • Conduct wellness assessments: Evaluate residents' needs and develop personalized wellness plans across physical, social, cognitive, spiritual, and mental health domains.
  • Coordinate with healthcare providers: Collaborate with primary care physicians, mental health providers, and hospitals.
  • Partner with housing staff: Customize programs to meet resident needs and educate staff on resident health changes.
  • Resident file management: Complete health information, end-of-life planning, and emergency contacts.
  • Support services linkage: Follow-up with at-risk residents to ensure adherence to health and wellness activities.
  • Relationship building with payers: Develop and maintain relationships with organizations serving seniors.
  • Effective communication systems: Ensure seamless information relay between housing and providers regarding residents' conditions and transitions.
  • Promote self-care: Offer personalized coaching, goal setting, and program coordination to empower residents.
  • Referral coordination: Connect residents with local services like Visiting Nurses, Rehabilitation services, Memory Evaluation, and more.
  • Care planning: Lead or participate in care planning meetings at housing sites.
  • Dementia needs assessment: Evaluate and refer residents as needed.
  • Program implementation: Collaborate with the Nurse Care Manager on evidenced-based programs.
  • Discharge planning: Work with hospitals, Home Care, and VNA for safe discharges and adequate services.
  • Transition support: Aid residents moving between care levels, coordinate family meetings, and participate in team discussions.
  • Advocacy & liaison: Act as resident advocate and family communication point.
  • Metrics & reporting: Track and report outcome measures to gauge program effectiveness.

Qualifications

  • Education: Master’s degree in social work or relevant human services field with 1-2 years of case management experience; or Bachelor’s degree with 3-5 years relevant experience.
  • Passion: Compassionate with a desire to serve senior populations.
  • Skills: Excellent organization, interpersonal, and communication skills; proficiency in Windows, Word, PowerPoint, Excel; ability to manage multiple projects proactively.
  • Additional Requirements: Fluency in Spanish is mandatory; travel within Boston metro area; dementia care certification preferred.

Salary Range 💰

$53,971.45 - $80,957.18
