About HSL & Our Mission 🌿
HSL provides enhanced housing equipped with services for seniors across four affordable locations and partner sites. Our proactive approach involves regular resident engagement focused on health, wellness, and independence, supported by dedicated resident services staff.
The Role of Wellness Coordinator 🩺
The Wellness Coordinator leads the R3 program, actively engaging residents through wellness assessments, health education, and connection to essential services. Responsibilities include:
- Conducting wellness assessments and developing personalized wellness plans.
- Coordinating with healthcare providers and connecting residents to needed services.
- Working closely with staff, including Nurse Care Managers, to coordinate activities and interventions.
- Ensuring thorough documentation and tracking of all resident interactions and interventions.
- Maintaining effective communication with partners such as emergency responders, healthcare payers, and local providers.
Location & Language Requirement 📍🗣️
This position is based on-site at a vibrant housing community in Boston. Fluency in Cantonese is essential, as the majority of residents speak Cantonese.

Key Responsibilities 📝
- Perform wellness assessments to identify needs and craft tailored wellness plans covering physical, social, cognitive, spiritual, and mental health areas
- Collaborate with primary care, mental health providers, and hospitals
- Partner with housing staff to develop resident-specific programs
- Educate staff on recognizing and communicating changes in residents’ health
- Manage resident files, including health info, end-of-life planning, and emergency contacts
- Follow up with at-risk residents to ensure adherence to wellness activities
- Establish relationships with payers serving seniors and streamline communication systems
- Promote self-care through personalized coaching and coordinate with care providers
- Make appropriate referrals to local service providers such as Visiting Nurses, Adult Day Programs, and Memory Evaluation services
- Lead or participate in care planning meetings
- Address the needs of residents with dementia and make necessary referrals
- Collaborate with Nurse Care Managers on Evidence-Based Programs
- Coordinate safe hospital discharges and transition to other care levels
- Advocate for residents and serve as a liaison with families
- Track metrics, evaluate outcomes, and prepare reports
Qualifications & Skills 🎓
- Master’s degree in social work or a related human services field plus 1-2 years of case management experience in aging services (preferred); or Bachelor’s degree with 3-5 years of relevant experience
- Genuine compassion and a strong desire to work with seniors
- Excellent organization and interpersonal skills
- Proactive, professional, collaborative, and results-driven mindset
- Positive attitude, excellent communication skills, sound judgment
- Motivated to learn and adaptable to change
- Strong documentation skills using AASC Online software
- Computer literacy: Windows, Word, PowerPoint, Excel
- Willingness to travel within the Boston metro area
- Preferred: Certification in Dementia Care
- Fluency in Cantonese is mandatory
